There are three ways you can cancel your appointment.

Option 1: Log in to your NHS App and cancel the appointment (and soon you will be able re-book an appointment) all at the same time.

Option 2: Please telephone the surgery (01923 776295) and speak to our reception team.

Option 3: Complete the Cancel Appointment Form (see below).
