Hypertension is a common problem that can be largely managed at GP surgery level. Annual monitoring, treatment and control of patients blood pressure has proven to significantly reduce the incidence of stroke and chronic heart conditions.
We call all our hypertension patients annually to attend the surgery for review. The practice annual year for reviews runs from April to March each year. This year to minimise the spread of Coronavirus we are asking all our hypertension patients to complete the Home Blood Pressure Monitoring form.
The questionnaire when received will be reviewed by a GP or our clinical pharmacist and if necessary they may call you to further discuss your blood pressure or to review your blood pressure medication.
If you do not have a home blood pressure monitor it is worthwhile buying one.
If you do not wish to or are unable to buy a home blood pressure monitor you can call the surgery and book a hypertension review with a nurse. We have set up what is known as a “cold floor” where we are carrying out patient face to face reviews as per normal. We have a separate entrance to this floor and we are vetting patients prior to booking them in for a face to face appointment to make it as safe as possible for patients to attend the surgery for their reviews. As per government guidelines please ensure that you wear a face covering to your appointment.
