How to Register
If you live in the practice catchment area (please click on link below to see a map of The Colne Practice catchment area)and wish to register at the practice please pick up the registration forms from the surgery, or alternatively you can register online.
Wherever possible members of the same household will normally be allocated to the same doctor. We try to maintain balanced lists by age and gender amongst the doctors.
Acceptance on to the list is as at the discretion of the Doctor. Please note that your medical records may take several weeks to arrive at the Practice.
Please note that if you were previously registered with a Doctor outside the UK then the Practice will not have access to your medical notes. All patients who register from abroad should contact their previous Doctor to obtain copies of their notes.
Not in our catchment area?
There are a number of reasons why you may not be able to register with your chosen GP, for example you may live too far away. If this is the case simply choose another GP in your local area.
Register online
We are using an NHS online service called Register with a GP surgery that makes it easy to register with this GP surgery. This secure system will email us your registration form.
Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number.
The service is designed and run by the NHS, so your personal information is safe. It reduces our administrative workload and makes it easier for you to register.
Paper forms are still available if you need one.

The National Care Record Service (NCRS)
Important information regarding your NHS medical records – Summary Care Record
The National Care Record Service (NCRS) is the way in which health professionals now access the Summary Care Record (SCR). The SCR has not changed, it is just the way SCRs are accessed that has changed.

Organ Donation
