Introducing Blended Triage at The Colne Practice

Effective from 1st February 2025

To improve patient access and ensure appointments are allocated based on clinical need, we are introducing Blended Triage for urgent appointments with GPs, advanced nurse practitioners, and other clinicians. Routine appointments with GPs, nurses, physiotherapists, and pharmacists will still be available to book via reception.

How It Works

  • All urgent appointment requests will be assessed by a GP before booking.
  • Patients must submit an Online Consult Form via our website.
  • If you call or visit the practice, our reception team will assist you in completing the form.
  • A clinician will review your request and direct you to the most appropriate care pathway.

What Happens Next?

Once your form is reviewed, you may:
✔ Receive an appointment booking via text or phone call.
✔ Be asked for more information or photographs.
✔ Get signposted to another appropriate service.

Why the Change?

Increasing demand for appointments means we need a system that prioritises clinical need while reducing waiting times and telephone queues. Blended Triage will ensure urgent cases are seen promptly while maintaining access to routine care.
